Claims Administration

In order to be eligible for compensation under the Settlement, you must complete a Claim Form. All claim forms must be postmarked on or before December 1, 2004 to participate in the Settlement.

On-Line

Claimants are able to complete the necessary forms on-line though the on-line claims administration system. This system will assist claimants in the completion of their forms to ensure all necessary information is submitted to the claims administrator. This is a secure site that, for security reasons, requires both your BC Registration Number and Deloitte File Number be used to log onto the system. The on-line system allows claimants to complete the necessary forms, change addresses as appropriate and submit questions to the claims administrator.

If you are a Solicitor, acting on behalf of a claimant, you may also register on-line. You will be able to register once to become an on-line claims administration user and then complete the Solicitor's Certificate of Legal Advice on behalf of your client(s). If you have the authority from your client, you will also be able to complete the Claim Form on behalf of your client(s) on-line and use all elements of the on-line claims administration system available to claimants.

Physicians are also able to register on-line to become a "Registered Physician". Being a Registered Physician will enable the physician to complete the Supporting Medical Form on-line for their patients. This segment of the system is designed to assist claimants and their physician(s) in completing their claim with respect to Supporting Medical Documentation and the claimant's Designated Medical Condition and Severity/Disability category.

    

Fax or Mail

To submit a claim form either via fax or mail, please download, print and complete the claim form, available here. Once completed, please fax or mail the form to the Claims Administrator, along with any required supporting documentation.
